Possible causes of this error are:
1. Corrupted or missing Phone.apk
2. A crash in Phone.apk while attempting to display caller ID and other information.

Method 1: Ultimate Caller ID Screen HD
Download and install Ultimate Caller ID Screen HD HERE
Method 2: Clear Phone data and cache
* Navigate to Settings > Apps > All > Phone
* Clear data and cache
* Reboot the phone
Method 3: Replace Phone.apk
Download the Stock ROM HERE and flash only
system.img (Android) using SP flash tool.
